We find it very important to educate you on procedures and give you the best cost estimate of a procedure before performing. Giving you an estimate and the knowledge of the procedure allows you to make an informed decision on the care you want to receive.
We are here to help you with all of your dental needs, but we need to enhance communication and promote your understanding of our Financial Policies. It is important to read through our policies, but please don’t be afraid to reach out and ask questions if you have any.
Financial Policies
Madison Ave Dental is happy to work with all patients who have dental insurance. To maximize your benefits, we will work with your dental insurance carrier and bill them directly for reimbursement of your payment.
When coming into our office and in need of a procedure, we will give you our best estimate of what your insurance can cover before the procedure. We want to make sure that you can make an informed decision on the care you’ll be receiving.
However, it is the patient’s responsibility to know the limitations and the benefits that are associated with the individual dental insurance plan.
Payment Options
Our office requests payment in full on the day of service unless specific arrangements have been made in advance.
If you have insurance, we will submit the claim to your insurance carrier. Your estimated out of pocket will be due at the time of service. After insurance pays their portion, we will send a statement to you if there is any remaining amount unpaid by your insurance. It is the responsibility of the patient to know the plan benefits and limitations associated with their individual insurance plan. The patient is financially responsible for all charges, whether or not paid by insurance.
A $35 fee may be assessed for any check returned by a bank.
There will be a $50 fee for any appointment canceled or rescheduled with less than 48 hours of notice. Please see our cancellation policy for more information.

CareCredit
At our office, we also offer Care Credit as a form of payment. Care Credit works with us to give the patients the dental care that they need. Using Care Credit can offer the patient interest-free payments on any procedure including, checkups, Invisalign, and teeth whitening.
